DENVER, Colo. – April 9, 2008 – Altitude Edge Consultants, a health care consulting firm, has been selected by Harbor Health Services, Inc. in Dorchester, Mass., to initiate a PACE program. The new PACE program will be called Elder Service Plan of Harbor Health. PACE (Program of All-Inclusive Care for the Elderly) programs are Medicare and Medicaid-certified services that involve a team approach to caring for a senior living at home in the community.

As a PACE provider, Elder Service Plan of Harbor Health will provide an integrated network of preventive and supportive care for seniors so they can remain healthy and in the least restrictive environment. Altitude Edge Consultants will complete a risk assessment and adjustment study for the program.

Harbor Health Services operates the Geiger Gibson Community Health Center and the Neponset Health Center in Dorchester, Mass., and the Mid Upper Cape Community Health Center in Hyannis, Mass. In addition to the Elder Service Plan, Harbor Health Services administers the Dorchester WIC Nutrition Program, the Ellen Jones Community Dental Center and the Columbia Point Infant/Toddler Child Care Program.
